A deposit of 50% of your session package fee is due to secure booking. While your session date will be tentatively scheduled on the books, it is not considered booked until your deposit has been received. Deposits can be made by PayPal or mailed check. (INITIAL BELOW)

Payments via Venmo @Stephanie_Gustafson82, or Paypal (Friends & Family PLEASE) can be sent to Stephanie.gustafson@yahoo.com, or mail can be sent to 6348 Redhead Way, Fontana, Ca 92336. Please let me know when you have your deposit payment so I can be on the look out for it!

Please arrive 10 minutes prior to session time to ensure preparedness. Children should be awake, dressed, and ready to go at designated session appointment time. (INITIAL BELOW)

I understand that all images taken by Stephanie Gustafson, Simply Captured by Stephanie Photography, are protected by copyright laws and that it is illegal to scan, copy, sell, print, or reproduce these images without permission. I understand that upon receipt of digital images, I will be given release to reproduce these images for personal use. (INITIAL BELOW)

Upon post production of images, I grant Stephanie Gustafson, Simply Captured by Stephanie Photography, the right to use and publish images for the purpose of display, editorial work, portfolio, website, or publication to promote Simply Captured by Stephanie Photography (INITIAL BELOW)

I understand that I will receive high-resolution, fully edited, digital images within 3-4 weeks of session date. I understand that although images may be received earlier than 2 weeks, it is not guaranteed unless expedited turnarounds have been agreed upon. Unless agreed upon prior to any editing work, high-resolution images may contain a mixture of color, black and white, and/or artistic edited images. Color correction, artistic editing, and/or retouching are included at the photographer’s discretion. Client may request further changes or alternately colored versions for an additional charge, after receipt and review of the high-resolution images. (INITIAL BELOW)

CANCELLATIONS: NO REFUNDS WILL BE GIVEN. If your session has to be cancelled due to weather, I will do my best to reschedule the session. The deposit will be applied will be applied toward a future session. I will determine if the session will be cancelled up to the last minute of the session. (September_-December) If client cancels within 14 days, the session cannot be rescheduled.

PRINTING: I understand that along with the digital files, I will receive a print release allowing me to print unlimited prints of any images given. I also understand and agree that Simply Captured by Stephanie Photography and /or its photographer are not responsible for quality of any prints not directly ordered through Simply Captured by Stephanie. Prints purchased directly through Simply Captured by Stephanie photography are guaranteed for quality (including clarity, cropping, colors, etc) INITIAL BELOW

ARCHIVING: I understand that my digital images will remain on archive with Simply Captured by Stephanie Photography for a limited time. Simply Captured by Stephanie hold no responsibility for maintaining your images digitally in excess of 1 year. (INITIAL BELOW)

MODEL RELEASE: I hereby give permission to Simply Captured by Stephanie to use my photos and likeness in all forms and media for advertising, portfolio, demo, trade, stock photography, editorial, altering without restrictions, and all other lawful purposes. I understand I am entitled to no compensation. I release the photographer of all forms of claims and liability related to my photo usage. I am over 18 years of age, and I fully understand the terms of this release.

COVID Liablilty/Release: I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.I further acknowledge that Simply Captured by Stephanie has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.I voluntarily seek services provided by Simply Captured by Stephanie and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.I attest that:* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.* I have not traveled internationally within the last 14 days.* I have not traveled to a highly impacted area within the United States of America in the last 14 days.* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.I hereby release and agree to hold Simply Captured by Stephanie harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act, or that may otherwise arise in any way in connection with any services received from Simply Captured by Stephanie. I understand that this release discharges Simply Captured by Stephanie from any liability or claim that I, my heirs, or any personal representatives may have against Simply Captured by Stephanie with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Simply Captured by Stephanie.

Covid Notification/Reschedule: I will notify Stephanie IMMEDIATELY of any changes in health/symptoms 10 days before, 24 hours before, and any time after the session within a 14 day post period. Should a diagnosis appear on my behalf or yours, your session will be rescheduled when safe to do so. (Initial Below)
